COVID-19 Guidelines & Screening Questions
In light of the pandemic, we’ve further strengthened our office protocols and infection control procedures to ensure that your visit remains safe and pleasant.
We’re now open for all treatments, including routine check-up and cleaning.
Patients will be required to complete a COVID-19 questionnaire prior to arriving at the office to help us screen for COVID-19 symptoms
Patients will need to arrive on time for their appointment, no more than 2 minutes prior, to reduce crowding within the office
We require masks in the office. If you forgot to bring your own, we’ll provide you with one.
Extra companions will be limited to only essential people in order to minimize crowding.
For everyone’s safety, we ensure minimal overlap in the hallways by offering staggered appointments, so the office will feel calm and quiet.
Use shoe covers provided at the office entrance to cover your footwear and apply hand sanitizer
Our staff will take your temperature upon arrival to confirm absence of fever – patients with a temperature of over 100.4° F will be asked to reschedule.
Maintain social distance and stay six feet apart from fellow patients and office personnel as much as possible for everyone’s safety.
Use hand sanitizer upon entering and use anti-viral oral rinse to reduce the viral load in the mouth before and after treatment is performed.
To reduce trips to the office and germ exposure while in transit, we will try our best to offer you with options for same-day treatment when applicable.
Notify Golden Mile Dental if you test positive for Covid-19 within 2 weeks following your appointment.
All of our clinical equipment has always been and will continue to be either single-use or thoroughly sterilized.
Office surfaces will be disinfected throughout the day to create a more sterile environment
Our clinical team uses appropriate personal protective equipment (PPE) such as N95masks, gowns, face shields among others to reduce the risk and spread of infection.
Air purifiers have been installed in the office to remove droplets and aerosols from the air, keeping the office air clean and safe.
Our team will participate in daily screening, which will include answering health screening questions and having their temperatures taken.
We are closely monitoring the guidance provided by RCDSO as they update their guidelines, we will update our safety practices in real time.

COVID-19 Screening Questions
Please fill in the following information. Your answers are for our records only and will be kept strictly confidential subject to applicable laws. You will be asked some questions concerning COVID-19 and your overall health. We only ask for information that is absolutely needed for us to protect our patients, staff, and community.
1. Please wear your own mask all the time when you are at the clinic( if you don’t have one please inform us, so one would be provided to you)
2. Please perform Hand Hygiene (washing with water and soap or sanitizing with hand rubs) UPON Arrival & After the appointments.
3. Please inform the office if you experienced any symptoms of Covid-19 within 14 days after your appointment.
